Admissions take place after declaration of NEET results and XII Std. results
All students seeking admission shall apply in the prescribed form issued along with the prospectus issued by MH-CET. (Refer MHCET brochure for detailed information.)

12 Medical Fitness as per (Annexure G) (On letter head of Doctor)
13 Medical Haemogram report.
14 Physical Handicap certificate (If applicable)
15 Copy of gazette for change in name (If applicable)

17 HSC passing certificate
18 Copy of preference form
19 Birth certificate
20 Undertaking for fees on Rs. 100/- stamp paper (Annexure F) [as per format attached] notarized.
21 Undertaking for ragging by student on plain paper. [as per format attached]
22 Undertaking for ragging by parents on plain paper. [as per format attached]
23 Passport size photograph (02 no) guidelines mentioned below.
Candidates are supposed to register at http://muhs.digitaluniversity.ac/ and activate their e-suvidha account. Kindly follow the instructions mentioned in the site. Candidates are expected to preserve their username, password, challan no.
This is for verification at institute level. Candidates will have to provide uploaded scanned copies of following documents. Hence, it is advisable to carry them in jpeg. In pendrive Format (for verification purposes).
